If you have experienced a problem with a taxicab ride, driver, or company;  your first step should be to contact the taxicab company directly to try and resolve the issue.  If you cannot resolve the problem directly and wish to report it to OCTAP, please feel free to use the forms provided below.  When filling out the forms, please fill them out completely; and also remember to state the outcome you are seeking (i.e., reimbursement, etc.); also include your email address.
